The purpose of this survey is to collect data about past students. This information is used by the Ohio Department of Education as a state wide picture of where graduates are at this point. It will also become part of our school and district state report card that is published each year.
Please answer each question to the best of your ability. There will probably be questions that do not pertain to you, if that is that case please mark "Not Applicable" .

Attention Maysville Families,
It has come to our attention that some families did not receive their open enrollment letters. We sincerely apologize for this oversight. Please rest assured that we would love to have your students in our district. To check the status of your child's open enrollment, you can call the administrative office at 740-453-0754. Office hours for the Administration Office are 8:00 AM - 3:30 PM until August 5th, and then 8:00 AM - 4:00 PM after that date.
Additionally, we understand that some elementary school students did not receive their teacher assignment letters. We apologize for any inconvenience this may have caused. Our elementary school secretaries will be back in the office on August 1st. You can call on or after this date to find out your child's teacher. Office hours for Maysville Elementary School are 8:00 AM - 3:30 PM.
We are currently still accepting open enrollment students. To enroll your child in the Maysville Local School District, please use the link below or visit our administration building to obtain a copy of the open enrollment form.

2024 Reserved Football Seats for 2023 Reserved Football Seat Holders are now on sale. Those who had seats last season should have received an email with the option of renewing those seats for this season from HomeTown Ticketing. The instructions are in the email to renew them. If you have questions, please email me at pat.miller@maysvillelocal.org or call 740.454.7970 and leave a message and I will get back to you.
Those who wish to purchase new seats for the 2024 Season will go on sale around August 4th.
